Office Staff may use and disclose medical and financial information related to your care that may be required to facilitate payment by third parties for services supplied by us, or to assist with, or facilitate the accumulation of information for purposes. Such information may be released to insurance companies, HMO’s and PPO’s, managed care organizations, IPA’s, Medicare/Medicaid, or any organizations contracting with any of the above entities that work to complete such functions. A doctor that is responsible for your medical care or the payment can have your medical records delivered to them.

Outside of the guidelines previously mentioned, without your authorization we will not disclose any of your medical or financial information. You hold the right to withhold your authorization of the use of your personal information at any time.If you have a requested restriction, our medical office is not required to correspond.
